TransforMED's professional facilitators work with your practice to target, plan and implement the improvements your practice and care team need to achieve Medical Home designation.

Full Transformation assesses and engages the practice in every area of the The TransforMED Patient-Centered Model. An on-site Baseline Practice Assessment evaluates your practice's performance in each of the Model's modules and culminates in a detailed report. Based on this assessment, your Practice Enhancement Facilitator (PEF) will work closely with you to custom design a program of evaluations, education, training and metrics — a deep and honest exploration of your professional goals and the coordinated actions required to accomplish them.

Annual costs, number and duration of of on-site visits and dedicated facilitator time are dependent upon the size of the practice and scope of service. Please CONTACT US for detailed information.

Full Transformation includes:

• On-site Baseline Practice Assessment with written report
• Dedicated facilitator time each month
• Email communication with your facilitator
• Weekly conference calls with various people in the practice (physician leader, practice manager)
• Metrics as determined by practice needs
• Yearly site visit(s)
• Opportunity to participate in group webinars on Patient-Centered Medical Home Model
• Opportunity to participate in regional Learning Collaborative meetings at a reduced cost
