Baseline Practice Assessment
The
Baseline Practice Assessment is a combination of proprietary assessments
and commercial evaluation tools administered by certified facilitators
during on-site observations and data collection. The
goal of the assessment is to identify a baseline for the practice as it
relates to TransforMED's Patient-Centered Model. During the
visit, one-on-one interviews are conducted with key stakeholders in the
practice. Work-flow and office operations are observed and team interactions
noted.
Leaders representing
clinical and administrative roles are identified in order to obtain a
broad view of the practice’s strengths, weaknesses and work styles.
Prior to the on-site visit, key members of practice leadership are asked
to complete TransforMED’s online Medical Home Implementation Quotient
(MHIQ). In addition, all practice staff are requested to complete a confidential
online Clinician Staff Questionnaire (CSQ) to provide feedback on perceptions
of leadership and practice culture.
Based on analysis
of the assessment results, on-site visit observations, interviews, phone
conversations and e-mail communication with key staff, your Practice Enhancement
Facilitator will provide your practice with a report that includes Areas
of Strength, Opportunities for Growth, MHIQ Scores(s) and Points for Consideration
related to each of the modules of the Patient-Centered Medical Home Model. Baseline Assessment costs vary by practice size and engagement type, but typically range from $1200 to $4800.
