Centers of Excellence Consultant - Collaborative PCMN CMMI Project

Kathy Gramlich-Muehlbach serves as Centers of Excellence Consultant for the Health Care Innovation Award (HCIA) from the Centers of Medicare & Medicaid Innovation at TransforMED. The HCIA was granted to TranforMED to demonstrate the Patient Centered Medical Neighborhood (PCMN) model of care to expand integration and the coordination of care in the medical home model.

Kathy came to TransforMED from VHA, Inc. working in Performance Services with  Clinical Value Analysis  responsible for assessments, strategic planning and value analysis design with the supply chain of members. In addition, Kathy spent two years as the Director of Clinical Performance at VHA Mid America region conducting CMS and Joint Commission mock surveys and assisting members in their improvement efforts with value based purchase measures.

Kathy has over 20 years of nursing experience primarily with surgical services, critical care and staff development. Particular areas of expertise include quality, performance improvement, facilitating teams and strategic planning.

Kathy received her undergraduate degree in nursing at Avila University and master's degree in management and education from Baker University. She is certified in Six Sigma Green Belt from Villanova University.
