NDP Update

The National Demonstration Project (NDP) concluded in June of 2008, except for the evaluation component, which is ongoing. Read the most recent Evaluators' Report.

At the conclusion of the NDP facilitation component, nearly all of the participating practices, both facilitated and self-directed, expressed an interest in maintaining the momentum and the relationship network to continue their improvements and validation measurements of the medical home. To accommodate them, TransforMED inaugurated the Touchstone Group, a virtually facilitated relationship network and Medical Home work group.

The Evaluation Team's final report on the project is expected in 2009/2010.
